Failed to appear for a previously scheduled exam
Were denied access to the previously scheduled exam for presenting yourself more than 30 minutes after the scheduled start time
Were denied access to the previously scheduled exam for lack of proper identification

If you previously failed your ProCertification computer-based exam, do not complete this form. You will need to complete the NRCA ProCertification Computer-based Exam Retesting Application.
